Short answer
Astigmatism is managed daily with glasses or toric contacts, or corrected permanently with LASIK (up to ~5 D Cyl), SMILE Pro (myopic astigmatism only), toric ICL (up to ~6 D Cyl), or PRK. The best option depends on your Cyl, combined Sph, corneal topography, and thickness. Keratoconus must be ruled out before any laser.

Treatment by Cyl level
- 1−0.5 to −1.5 D (mild): glasses, toric contacts, or LASIK/SMILE if bothersome.
- 2−1.5 to −3.0 D (moderate): LASIK and SMILE excellent; toric ICL alternative.
- 3−3.0 to −5.0 D (high): LASIK with careful Axis alignment; toric ICL if cornea borderline.
- 4Above −5.0 D (very high): toric ICL primary; topography mandatory.
Astigmatism plus myopia or hyperopia
Combined prescriptions (e.g. −4.00 / −2.00 × 90 or +3.00 / −1.50 × 180) are corrected in one procedure — LASIK, SMILE (myopic only), or toric ICL. Astigmatism does not add a separate surgery.

When surgery is not suitable
- Active or progressive keratoconus
- Irregular astigmatism on topography
- Unstable prescription (<12 months)
- Pregnancy or under 21
- Regular astigmatism on Pentacam
- Stable 12+ months, age 21–55
- Adequate corneal thickness for laser
