Short answer
Astigmatism distorts vision at all distances — caused by uneven corneal curvature, measured by Cyl on your prescription. Hyperopia (long-sightedness) blurs near vision first — caused by the eye being too short, measured by positive +Sph. They are different errors but often combine (e.g. +2.50 / −1.50 × 90). Both are correctable in one LASIK or toric ICL session.
Astigmatism vs hyperopia at a glance
| Feature | Astigmatism | Hyperopia |
|---|---|---|
| Prescription field | Cyl + Axis (e.g. −1.75 × 85) | Plus Sph (e.g. +3.00) |
| Blur pattern | All distances — smeared, doubled | Near first; distance OK when mild |
| Night vision | Starbursts, streaking | Less affected unless moderate+ |
| Headaches | Eye strain from distortion | Accommodation overwork on near tasks |
| Can combine? | Yes — very common | Often with Cyl on same prescription |
| SMILE Pro | Myopic astigmatism only | Not for hyperopia |
| LASIK | Up to ~5 D Cyl | Up to ~+5 D Sph |
How to tell which you have
- Distance and near both smeared, Cyl on prescription → astigmatism (± combined Sph).
- Reading strains, headaches, +Sph, no Cyl → likely hyperopia alone.
- Near blur after 40 with +Sph → hyperopia plus possible presbyopia. See Hyperopia vs presbyopia.
- Distance blur, near fine, minus Sph → myopia, not hyperopia. See myopia guide.
Combined hyperopia and astigmatism
A prescription like +3.00 / −1.50 × 180 means +3.00 D hyperopia with −1.50 D astigmatism — both corrected by LASIK or toric ICL in one procedure. SMILE cannot treat this because Sph is plus (+).

Treatment paths compared
Astigmatism alone: LASIK, SMILE (myopic only), toric ICL. Hyperopia alone: LASIK or ICL — not SMILE. Both together: LASIK or toric ICL — one session, one recovery.
